The Test Request Form provides ITEL the information required to complete a sample analysis and return results to you. The same form can be used for flooring pricing and roofing and siding matching.

Required Information To Complete an ITEL Test:
- Your first and last name
- Phone number where ITEL can contact you in case we have questions about the sample
- Contact e-mail or fax where the reports should be sent
- Claim number and insured’s name
- City, state, zip code of the loss (city, province, and postal code for Canadian tests)
ITEL Service Times
Flooring Pricing
(includes carpet, pad, vinyl, laminate, ceramic and stone flooring tests)
- Regular Service: The lab report will be sent next business day after the sample was received by ITEL.
- Express Service: The lab report will be sent the same day the sample was received by ITEL. Express service is not currently available for ceramic, porcelain, and stone tile or asbestos analysis.
Roofing Match and Locate
- Regular Service: The lab report will be sent the next business day after the sample is received at ITEL. Express service is not currently available for roofing matching.
Siding Match and Locate
- Regular Service: The lab report will be sent the next business day after the sample is received at ITEL. Express service is not currently available for siding matching.
