How to Collect a Vinyl Sample

1. Collect a 2” x 2” vinyl sample, preferably:
   1. the cleanest, least damaged piece you can find,
   2. from a low traffic area such as the corner of the room,
   3. then place it in a small plastic baggie.
2. If Asbestos is suspected and an asbestos test is desired, please collect three random samples using the SampleRight™ collection kit provided free-of-charge by ITEL (order kits online). Remember to collect three samples of each color of the flooring if different colored tiles are present.

If there are multiple layers of vinyl flooring in one room, please clearly indicate which layer corresponds to the Test Request Form and the type of test required. Use one form for each layer, as each layer will be separately tested.

Once you've collected the sample, fill out the ITEL Test Request Form.
